There was a known issue occurring between WP 2FA and the Toolkit Front End Login module, however, this issue was resolved in Toolkit version 3.8.0 (see https://ww.wp.xz.cn/plugins/uncanny-learndash-toolkit/#developers) and Melapress WP 2FA 2.9. If you are using these 2 releases (or newer if available), then the issue should no longer be occurring.
Also, if you are using Memberpress for the Login page, then this may be an issue related to Memberpress and not the Uncanny Owl Toolkit plugin.
Can you please confirm if you are using the Toolkit plugin for your login page?

I am experiencing a similar issue as described by the original poster on my site. This happens when I try to log in with an admin user name. We require 2FA for admin login. It just redisplays the WordPress login page. I am running version 3.8.0 of the Uncanny Toolkit for LearnDash and version 3.0.0 of the WP 2FA – Two-factor authentication for WordPress plugin. When I deactivate the Uncanny Toolkit for LearnDash plugin the problem does not occur. This past weekend I updated the Uncanny Toolkit for LearnDash plugin from version 3.7.0.4. I am not using Memberpress for the Login page.

We ran a test on our staging site by activating WP 2FA from the Front End Login module settings (we are using WP 2FA 3.8.0). We then confirmed that there were no specific user or user role exceptions in the WP 2FA settings to ensure that 2FA will affect ALL users attempting to log in. Finally, we tested a log in as an Admin and upon submitting the key, we are redirected to the wp-admin section.

I don’t have memberpress, but the same (or similar) issue is happening to me.
When the user logs in, instead of the page to enter the 2FA code, it takes them to a page that just shows the [uo_login_ui] shortcode.
This seems to have started in the last week as well, so I am assuming an issue with 3.8
