    That said…

    The shortcode you are using is deprecated (for quite some time now) and really it should not be used. Take a look at the doc page for the recommended setup:

    And, that said…

    If the buttons do not do anything, then the cause is nearly always a fatal javascript error from another plugin or the theme. Fatal javascript errors prevents any/all scripts after the error from running. To see if you have a fatal javascript error, load the page in Chrome, hit the F12 key and then click the Console tab. All browser errors including javascript error will be shown here.

    Just about always you can determine the source of the error by looking at the file name or by mouse hovering over the filename so you can see the file path.

    If it is another plugin causing the error, deactivating that plugin will allow Form to run correctly.
