Constant Contact Forms Issue

I’m getting this message:
Constant Contact Forms has experienced issues that may need addressed and functionality may be missing. Please enable the “Support” checkbox under the Support tab in Constant Contact settings and start a forum support thread. Our support team will aid with further steps.I’m seeing a “constant contact use field” in my sign-up form in the footer – please help with removing this.
I also want the message that emails are serviced by CC underneath the signup fields removed and editable by me (a paid customer).
I enabled debugging logging per the instructions above – please let me know what next steps are. Thank you!
