Contact form

The “Contact Form” on my blog doesn’t seem to be complete. I’m using the Suffusion theme, version 3.3.6. When I display the form, the boxes for the users basic information and message are displayed, but the “subject” box only displays this message to “Please make a selection”, but no choices are cited. Where do I go in the editing controls to provide these choices? I don’t see any such feature for the editing process. Also there is a box for a question and “Why ask?”, but there is no question cited to force the user to prove that he or she is a human being. Why isn’t a question cited? Where do I go to state one? When I go to the “edit” form for this page, the basic boxes are not displayed. Is that OK? I’m not sure that this page is setup correctly enough to be used.
