add the wordpress jetpack plugin and create a wordpress.COM account if you dont’t have one.. then in ‘edit page’ it should appear the option ‘add contact form’
In view of reported conflicts between Jetpack and Customizr, it might be prudent to try an alternative like Contact Form 7,Ninja Forms – or the rather jolly Sexy Contact Form.

Has anyone tried Formget with the tab slider for a contact form? If so, do you know how to add the Java script to certain pages (between <Head> and </Head> without changing the CSS template?
