Cool I have a question about it. I installed it and activated it. I edited the messages and email address, etc. But where do I actually create the form?
I opened up to write a page but not sure I see anything in there to create a form.

To your question above: Disable the WYSIWYG editor (Users -> Your Profile, bottom option, uncheck and save). Now, when on the page editor, you’ll see a new quicktag there to insert the contact form.

Oh ok, I see it. I added it and it looks great. Is there a way to add other options and customize it? How about offering drop down menu options etc?

wp-contactform was adding some css into every page on my site, so I changed the code so it wouldn’t do that and put the css into my style sheet. You might want to do the same if you want to avoid needless bloat.
The only other customization I did was to remove the “site” text box.
