It looks like you may have disabled the plugin you were using for the contact form. I think it looks like a Jetpack contact form shortcode. Do you / did you have Jetpack installed?

Im a rookie but Jetpack has screwed up my pop subscriber (gone) and now this.
I had and have reinstalled per others advice.
I never touch the site but every week something fails

So, check the Jetpack settings and make sure you’ve enabled the contact form.
