I need 2 WordPress solutions.

1. The site will have sales through a subscription. It’s about diet. Will Pekiet 1, Package 2 and Package 3. The client buys Package 2.

I need a plugin that allows me to add the customer’s first and last name. It will allow you to create custom fields, e.g. type of diseases, weight, height, diet, products. Additionaly would like to be able to mark the validity for a given customer. Let’s buy it for Package 2 and have 1 month of support so that I can see the date, eg start – end.
I would like to mark meetings with this client (integration with Google Calendar) or send him emails from WordPress directly SMTP)

2. Form with more than 30 fields (diet questionnaire) I need the date of the meeting to be added to the google calendar and information from this survey (name, email, name, diet) added automatically to the above.
