Conversion tracking not working

The tracking of customers seeing the tests is working fine, however sometimes it seems that the test is not recording some of the conversion results for some reason. The conversion result is set to be a thank you page, after form submission. Sometimes it is tracking well the conversion and sometimes not. Not sure what is causeing it not to record sometimes…
