Cosmetic bugs, wrong Plugin URI, readme

In file media_author.php line 4:
Plugin URI: http://wordpress.com/extend/plugins/media-author/
should be changed to ..ww.wp.xz.cn/.. or the link from plugin list to plugin homepage is broken.

And file readme.txt is not updated to reflect 1.0.4 changes.
