Ah, i see that you have an issue and i didn’t contacted you, sorry for that. Please use this link when you have an issue https://smartforms.uservoice.com, i check that site constantly, the problem of contacting me in other ways is that the notification will go straight to my email and since i receive a lot of email daily it might take me a lot of time to reply, sometimes i even forget the email and i never reply which is terrible but i am only one person and i sometimes make stupid mistakes, submitting a ticket in that site allow me to better follow up issues and give faster responses. There is also a support tab inside the plugin where you can submit support tickets directly, this is totally my fault, sorry.
Back to the problem, something else must be happening, submitting a license only give you more features, it doesn’t do anything to crash your form, maybe you also added a formula? if the formula is wrong your form could stop displaying.
Can you send me a link to the form? i tried your site but it says “Object not found” or if you are just tired of the plugin and want a refund (which i would totally understand) just let me know and i will issue it right away.
