Create electronic form for WordPress

I’d like to be able to create and electronic form to look like either a .pdf or Word doc. However I want my users to come to my page, fill out the form and the form and send and or print the form. When the form is sent to me via email I need it to come in a format that looks like a document or .pdf because it’s a contract, I have tried the Visual Forms but that doesn’t work as most of those plugins get emailed in text format, spreadsheet format or want to dump into a database. My end user will not know how to manipulate the date output so it needs to be in a simple document like format.
