Create form for customers via shortcode?

    Hi,

    is it possible to create a form for customers so they can insert their name, email, phone, address etc.

    There is a new shortcode now [wc_book_my_service] using this shortcode you can make your customers to

    – Select a manufacture
    – Select device related to manufacture
    – Select service
    – Add customer information

    And a job is started for that customer.
