Create front-end for file upload

I am trying to create a mockup of a web app, and need to have a front-end page where the user can upload a .csv file. I then need to:
1. Use the headers on that file to map to fields on a form.
2. Populate the form with the data from the file – one record for each row.

Any idea how I go about this? I am a total newbie at WP and was hoping to find a plugin to accomplish either parts of this process or all of it, but so far every plugin has been about uploading a file on the back-end. Any help would be GREATLY appreciated!
