Create special subscription form

    Hi,
    I am a new user of the newsletter plugin. The installation went fine. Now, I try to setup a certain subscription form with help of the documentation, but so far with no success.
    The subscription form should contains the attributes: email address, keyword, pre name, last name. The first two are mandatory, the others not. The keyword should be a field in which a user can write a keyword. To all users with the same keyword, I like to send emails.
    How do I need to setup the subscription form?
