Created fields – added shortcode – not all fields show

Hi Guys, Absolutely love the interface and being able to customise the field types. Came across a littleproblem though and its bugging the **** out of me.

I created the fields:
First Name
Last Name
Email
County (Drop Down List)
Type (drop down list)
Text Field (For location data)

All are set to required.

Created a WP Page, gave it a title and added a shortcode block.
Added the [pdb_signup] shortcode.

Previewed page, and only the First Name, Last Name and email input are showing?

Any ideas chaps, because once I get this part of my site functioning correctly, I have no problem making a donation to the cause.
