CSV importer and Google Map

Hello I want to use this topic :

But the CSV sample is not online and I cant donload the file
Can someone help me ?

The second line is an example of what goes in each column.
Col 1: Your Placemark Name,
Col 2: Your Placemark Address,
Col 3: Your Placemark Content (for the editor),
Col 4: bgmp
