Same issue here.
For me, in the body tag the main class is “admin-bar” instead of the “error404”.
So I am guessing its loading the wrong template.
In the debugger I can see that a proper 404 status was returned.
I get (what I think is) the same error as well:
/wp-admin/ takes me to a different version of my 404 page which gives a list of every single page on my website and then the 404 message at the bottom.
The list of website pages includes includes pages that aren’t in the menu or marked for search indexing and which I don’t want people to access unless they have a direct link.
This one thing is stopping me from using the plugin. It would be awesome if we had a solution to this issue.

I have a similar issue, using the Divi theme. /wp-admin just brings up a blank screen, no 404 page or anything.

I just updated to 1.1.6 and the problem is still there.
I am using a custom theme.
1.1.6 was not meant to fix this issue, and it might not be fixed because of how WordPress handles loading of /wp-admin/.
I’m considering removing the 404 error for any access to /wp-admin/ while logged-out and doing a redirect instead.
It appears to have fixed the issue for me.

Thanks for the update. Now instead of a broken page it shows a message:
‘This has been disabled’.
I prefer my theme’s standard 404 page to be displayed. I’ve found an alternative plugin that works better with my theme.
