Custom form

I created a single page php form that works on it’s own. Then I created a new page template within a WP theme and created a page via the admin area to use that template.

The page loads correctly and reloads correctly when there are errors in the submission. But once everything is correctly filled out, instead of displaying the message that the form was submitted, I get a 404 error. (The URL does not change.) Any ideas?
