I cannot reproduce this locally.
Just so we are on the same page, this is what I did.
1. Created a donation form and customized some of the texts in the form.
2. Using the Divi Modules plugin, I added the form to a page using the Divi Builder.
3. On my test site, the customized form shows in the Divi Builder and on the frontend.
If you’re not seeing the customized form in the builder preview, it maybe a cache issue. Publish the page and check if the form shows correctly on the frontend of the form.
If you are using a cache plugin or your host has some server-level cache try to clear it before you edit the page again.

Thank you for getting back to me with a resolution. Apologies for the delay, the notification had ended up in my spam folder.
I have managed solve the issue by using the Give Form Grid module for Divi. Now the multiple forms are displaying with the correct image and custom heading etc. However I set the Display format to redirect to the form page but the form opens in a Modal on the same page. What could be the wrong in this situation.
Please let me know.

This is a known issue. The workaround is to use the form grid shortcode instead: https://givewp.com/documentation/core/shortcodes/give_form_grid/
