Custom form not submitting

I’ve created a custom Profile Form and dropped it on a page using the shortcode provided. When I render the page on the front end, the submit button on the form does nothing. I looked at the source code and it doesn’t look like a form tag is being output at all.

Perhaps I’m going about this incorrectly. I’m trying to add new fields to collect when users manage their profile.
