Custom Form system

    Hi all, I am trying to find a contact form that is customisable in such a way to allow it to auto-generate a unique order number for each time the order is sent, ie AA001 for the first, and AA002 for the second use and so on.
    Also, it needs to send through the order in a way that is printable and neat – not the usual basic text style – see an example at http://bizzuk.com/files/form-example.jpg

    I also need to have 2 or 3 different versions of the form on the same WP site, so something like SI-Forms, that allow several would be great.

    I have hunted through all the contact form plugins, but cannot see anything that does this, unless I have missed one??

    This no longer needs to show/create a unique order number.
    So the issue now is to how to get the information that is created in the form to be sent via email in a formatted way, like that in the one in the link in my previous post.
