Custom Input field

First off, I love your plugin. I can’t wait for the Form Filler add-on though, because that will make this much easier. I have read through your tutorials, but I am new to coding, so I have a few questions.

1. How can I make the last name field mandatory in give_checkout_user_info?

2. This one is a little harder. How can I input a custom field like in the screen shot after the e-mail address box?

i.e. – <h3>Gift Recognition</h3>
Please recognize my gift using this name:

<label for=”name”>Your name:</label>
<input id=”name” class=”input” name=”name” type=”text” value=”” size=”30″ />
