Any list created using the list option appear as standard lists with what appears to be a broken image with each list item.

Apparently this appears like that in FireFox. In Chrome, I see the icon inside the list.

Fixed my issue. The problem was the HTML & XML section under the minify settings for W3 Total Cache. Disabled the “Inline JS minification” option and saved the changes. Then re-enabled and saved changes again.
