Custom Page Edit Fields

I am looking to add a custom Page Editor. What I would like is when one of my users presses “New Page” or “Edit Page”, be presented with a page edit with custom fields already filled out and the original editor not be available.

I would like for the pages to have these fields to fill out in the editor

Name
Information
Location
Notes

When the users fill them out, they are automatically added to the page that they created. When they click edit page, I would like the fields to already be populated.

Is this possible with Custom Fields? Or do I need to look into different plugins?
