The text that I now have in the custom structure is:
/%year%/%monthnum%/%day%/%postname%.html
but no .html appears on the end of the URI.
What could be the problem?

Was WordPress able to automatically update the .htaccess file after you saved, or did it provide manual instructions?
WordPress was able to automatically update the .htaccess file after I saved.
If I click on any of the common settings, the changes are implemented, it’s only the .html that doesn’t show up at the end of the custom structure.

Well, it looks right.
Now I’m wondering if a plugin or theme is interfering.
Try temporarily deactivating all plugins, switch to the Twenty Ten theme, and save your permalink structure again.
If that works, you should be safe to re-activate your plugins and switch your theme back.

I’ve deactivated all plugins and switched back to the twenty ten theme. I then saved the permalink structure but still didn’t work.
I also cleared the browser cache and then tried changing to IE just in case it was a browser issue – but to no effect.
Any ideas?
No, I think I’m fresh out of ideas. π
Are you using any caching plugins at all? If so, you sometimes have to delete the cache directory after deactivating the plugin.
