Custom Subject Line

Im no Code Guru,so I need help :). In your newest contact form your default subject line for e-mails is:
(Website): Form Submission to “(Form Name)”

I need to get it to just be: (Form Name)

Or althernativly have a Subject Text Box or Drop Down?

I also need to insert a XML at the end of the form for our Ticketing system. I attempted to do this with the “HTML” but it just keeps Vanishing… It also needs to contain the e-mail from the e-mail box in the form
