Before I purchase a premium plugin, I would like to know if this plugin allows to add custom thumbnails to the youtube videos? Is it an option? if not, can it be extended to allow that?

Right now, the video channel owner can customize the thumbnail on YouTube.com. The plugin will then apply that change.
