Daddy Analytics not showing

It’s collecting Lead info perfect, but none of the Daddy Analytics is showing.

I’ve confirmed my account ID and Tokens in settings. I have the javascript on the page, but I also noticed there might be 2 variations defining da_data: “var da_data = daddy_init(…” and “var da_data = init(…”

Do I need to create hidden fields for each of the daddy analytic fields I want to collect?
Anyone know what I’m doing wrong?
