Data collection plugin with database capability

I’m working an educational site that includes a weekly module for people to go through. They go through 8-10 pages of content including video, audio and text. I also need to ask them questions at different points throughout the module and collect the responses into a database. So ideally there would be 2-3 questions on each page. My question is can anyone recommend a good plugin or solution to collect this data (which is mostly text responses) in a way that will write it to a database. The person completing the module would be logged into WordPress with a username so it would need to collect the data for the logged in user so we know whos data it is. Any idea?
