Date picker is not working

Hello ,

I have used contact form 7 plugin . Now i need separate date field and time field which include pop facility that’s why i used date picker plugin but date picker theme is not working here. Also i tried to change date format mm/dd/yy to dd/mm/yy but it’s also not working.
I want the same date field and time like which is show in date picker theme but my form show date in simple calendar format and time show in drop-down format(0000:mm)
