I visited the page in question but I don’t see any such form there. There is some contact form on the “contact” page but that one doesn’t include date picker so I’m assuming it’s not the one.
Is there any specific page/area on site where we could see that form to test it?

Aside from that, such issue may be related to the datepicker field setting. For example:
– the “default date” being set to some date in December
– and the “Days of the week” under “Limits” tab set to “Selected days” but with no days actually selected.
If that’s not the case, then there may be some unexpected conflict that’s breaking the calendar but that would need to be checked.
So can you:
1. share a link to the page where we can see the form live on your site?
2. and also share form export with us?
To share form export:
– go to the “Forminator -> Forms” page in site’s back-end
– click on a little “gear” icon next to the form in question
– select “Export” option and either put given export code at https://pastebin.com service (as public paste) or export file at your Google Drive, Dropbox or similar account
– and include link to it (publicly available) in your response here.
Note: form export will NOT include any submitted data so those will safely stay in your site; we don’t need them anyway.

Thank you – we managed to solve this issue. The problem was the ‘default’ date was pre-picked and wouldn’t allow user to pick a new date.
