The equation in this case can be implemented as follows:

The equation is not working because a parenthesis is missing in the first IF:

var d = fieldname2+266-IF(fieldname4 == 'Day 3 embryo transfer', 3,IF(fieldname4== 'Day 5 embryo transfer',5,0));

return WEEKDAYNAME(d)+', '+MONTHNAME(d)+' '+DAY(d)+', '+YEAR(d);
