I turned off “Scale Images” setting in Lazy Load and it’s working fine now but I need this feature. It was working fine a couple weeks ago.

Right now I have turned off Scale Images.
Here are screenshot of what happens when it is active:

We noticed it started happening when 3 photos of the wrong size (smaller) were uploaded. They were fixed but now other images in the grid are shrinking (these are the proper size in media library though).

You previously mentioned that you have already cleared the cache from the Optimole settings, but make sure to have it done after you corrected the images from your Media Library.
If you are using a caching plugin, please delete the cache from there as well. Try enabling again the Scale Images option and see if it makes any difference.
No I’ve done that multiple times. I’ve cleared the image cache in Optimole then cleared the W3TC cache.
If I turn off Scaled Images it works fine. When I turn it back on after clearing caches it breaks again with the wrong sized images for some.
