Description Field does not show up

Thank you for a great form, it works well, however the description field text does not show up, it duplicates the field name instead. I have tried an older and newer version of the plugin on a few sites and it does not work anywhere. I can’t see anyone else having the same issue, which would seem standard. Are you able to assist please.
