In the Elementor editing view, my form is looking good. However, in the normal user view, my form seems broken. The button is not getting styled, and calendar drop-down is seems broken.
It seems there is a CSS style issue, however it was working fine previously.
Could you please let me know what would be the issue?

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.
You can share the export file via Google Drive, Pastebin, Dropbox, or any cloud service in the next reply.

I tested your form on my lab site and everything works as it should. It seems that the issue is with your site.
Were there any updates made on the site (plugins/theme)? If so, in the first place, I suggest running a full plugin conflict test. As issues exist only on that particular site, it must be something that made such a conflict.
Please deactivate all plugins except Forminator and check if the problem is gone. If so, then enable all plugins one by one and find which one is having a conflict. If there is no positive result, switch to the default WordPress theme like 2019 and see if it works.
Before this test, we recommend full site backup or running this test on the staging site.
