With this plugin, is it possible to display the attachment extension before title e.g. (pdf)Title of Attached file description

Sure. Relevanssi doesn’t really come into it – it’s all up to your search results template, and hasn’t anything to do with what provides the results. You can get the attachment file type with get_post_mime_type().
