Hi,
Our plugin wouldn’t add any code to your form by itself. The way the custom integration works is that you or your developer add the code to the form manually and that will then trigger the integration when the form is submitted.
Maybe if you share a link to where you see the box 3 times I can see what’s going on and if it’s even related to our plugin at all.

Thanks for such a quick response. The site is https://www.daverandall.art/.
You just need to pick a product and go to checkout. The Subscribe checkbox is from the MailChimp for Woocommerce plugin but there are actually 4 instances of a Mailchimp for WordPress v4.8.7 form in the source.
I have created a form using your plugin for use at the bottom of every page which is working fine.
The checkout subscribe is not working. I wondered if the plugins are interfering with each other.

The checkbox on the checkout is not created with our plugin so I can’t provide any support on that. There is no known conflict between our plugin and any other Mailchimp plugin, should work fine together on the same site.
There is a MC4WP form further down the site, but this has no relation to the checkbox for the checkout integration that you use in another plugin. And as you mention, the form created with our plugin works just fine.
Note that we do also have a checkout integration that you could use, it might just work. Otherwise I recommend you connect with the support department of whatever plugin added the checkbox to your checkout for you if it isn’t working. It won’t be related to our plugin.

in the end I deactivated and deleted both plugins and started from scratch. This time I hide the MailChimp for Woocommerce checkout form and used your checkout integration.
All works fine now. Thanks very much for your help.
