thank you for sharing these screenshots. The list of items shown on the images is not added by Flexible Shipping – it appears also when you use Flat Rate which is built-in in Woocommerce.
If you want to remove the list of items from the shipping box please get in touch with Woocommerce support.

Ok. Thanks for letting me know. I installed a bunch of woocommerce plugins at once and it is a little hard to track down what the source is.
