Display Categories, Published Date

I have looked through the support forums and haven’t found the answers, or only found them partially. I am comfortable with editing PHP inside the plugin files if necessary.

I would like to display the category and/or categories selected for the testimonial in the content of the testimonial itself.

I also wanted to display the date, but it looks like I got it to work using a solution posted earlier. Could I use a similar solution to display the categories inside the testimonial?

An additional feature that would be nice too is the ability for the client to select the category when they submit the form.
