'display forms' box hidden under 'form items' box

I was driving myself crazy trying to find the shortcode format to use with your plugin, when I saw a screenshot that didn’t look like my screen.

When I first go to the ‘add new’ screen, I see the ‘form items’ box. Only the top of the ‘display forms’ box is visible — not readable. If I scroll down to see the ‘display forms’ box, the ‘form items’ box jumps down and covers up the ‘display forms’ box. Of course, I didn’t notice that until I realized that there is supposed to be a ‘display forms’ box.
