You can report a post only once. It is done to prevent spam. To send another report, change email address in report form to something else. Or try another post.
Also, can you provide URL of your blog so I could check debug output? Thanks.
Also, I’ve updated the plugin a bit – there will be no more messages like this: “You have already reported this post before”. But each post can still be reported only once. If you want to test more reports, use another email address, or delete report in WP Admin/Reported posts.

Well, I had the message ON THE FIRST report. And my report page is empty.
I need your help with testing. Can you upload this updated version to your test WordPress and give me the url so I could check error output? This is very important for me.

Thank you for helping. In updated version, error message will not be displayed even if report was already sent. Also, there is logging turned on, so I will be able to see which error occurs exactly.
There was hard-coded table prefix (wp_) so it didn’t work for wordpress installations that had non-default prefix. This bug was fixed.
