If your forms were created before the installation of Save Contact Form 7, it might have been creating the problem.
Can you please let me know the version of WordPress and CF7?

Thing is I moved to Pods made CF7. Have absolut control over everything by it.:

so I forgot your plugin and this problem.
You and other few developers making plugins like this (even save to database plugin from CF7 developer self) shold mail each other or meet on Skype to agree about new CF7 shortode attribute, just for names of fields. As it is now it is not acceptable, and it was main reason I spent time to make it work with Pods.
You all are taking just shortcode names of fields and it looks stupid: text-345 for “Name”, textarea-333 for “Message”, etc.. I know you cannot do better before developer of CF7 gives you new shortcode attribute for names in your plugins. Something like this: save-database=”Phone”.

Regarding the field names, we are already using the actual name and not textarea-333 etc..

Please try Version 1.5, it shall work for you.
Works on localhost, only after making new form.
I had this problem using localhost as well, however it turns out that it occurs if you have loaded CF& first, used it, and then instal SaveCF7
I went to an existing form in CF7, opened it for editing, re-saved it and all works fine
