The plugin is not fully compatible with WP 3.5. WordPress changed the media handling in the new version, and the plugin has not yet been updated.
The plugin author commented in this thread.
(There is a way to get it to work, but you cannot use the “Add media” function. You have to use the box where you see “Set XYZ”, where XYZ is the label you have chosen for the new thumbnail, and navigate through that way.)

so it should work with WP 3.5? I can’t figure out how to upload an image with MPT
I have a box for featured image when creating/editing a post, but I can’t see a MPT box anywhere in my backend

What box? Where can I find it?
There is no MPT box for uploads. When you have an MPT registered and you click the link to set the thumbnail, it will present you w/the same thickbox that the featured image used prior to 3.5 where you can upload or choose an image from the library.
