It’s strange. For me it seems like shortcode is not inserted.
I can’t also find any code of upload form via chrome bug.
It can be that short codes of different plugins doesnt work on same page.
I have taken a screen shot where you can see I have inserted the code in visual editor

Yes, the whole contest plugin does only work when javascript is acitvated in browser.
Normally Javascript is always enabled in browser unless you deactivated it.
