Off the bat I see the issues with your csv file. You need to have them double quote encased. That means before
“each”, “entry”, “there”, “must be”, “a double quote”, “and, after, each”, “entry”, “as well”

You have some invalid headers: Company
and that’s with the first entry of the file you gave me with the doublequotes on everything:

Send me a screenshot or text (copy paste) of the Labels that are generated when you first choose the form you wish to import into. NOT the error report, the list of Labels, Id’s, etc… that the plugin provides to help you create your CSV file.
I’m pretty sure this is what you’re looking for:

Ok, I’m going to have to test. Haven’t been updating with newer versions of WP ’cause no $ coming in and I have priorities elsewhere, so could be compatibility issues with newer WP’s or newer Gravity Forms. I’ll test to see what is going on. Could be a day or two b4 I get back to you.
Just tested. Works fine. No issues whatsoever. Used the exact same file I sent you. Imported 103 entries.
Check all of your field names in the form. Make sure there are no spaces before or after the title. I’ve had people with this same issue (it’s always a csv structure issue) who had spaces before some of their titles. Go through and rename all of your fields by copying and pasting the titles from the csv into the form elements. Then try again. I just did it with the latest version of WP and the latest version of GF. If problems persist, I will ask you to export your form to me (not entries, just the form itself). Then I will import it into my system and test against that. If it continues, the you may have to provide me access to your site to troubleshoot. Let me know how the testing goes.
Well that did it for my testing. Even though there didn’t seem to be any difference (or spaces) in the form labels, I cut and pasted them in again from the csv, and that seems to have done the trick, though I don’t know why.
