After installation, used it on one of the posts. Everything seems to run fine, even the new category gets highlighted, but the permalink doesnt get updated.

Debugged a bit, it seems to update the wp_postmeta table correctly with the updated category for permalink – but permalink doesnt change 🙁

Use many plugins ( > 25), some of them are:

    Adminimize
    Advanced Custom Fields
    AJAX Thumbnail Rebuild
    Akismet
    Category Posts Widget
    Executable PHP widget

    W3 Total Cache

    25 plugins that’s a lot of plugins…

This plugin is using the only official WordPress method to get the permalink category so of course if any other plugin uses this system to hack the permalink as well, it might override it. This is impossible to test for me, except if I install all the plugins you have and try to disable one by one.

Do you have a LOCAL version of your website? If not, you should. Then disable all the plugins, except WP Category Permalink. Then try to see if the permalink for your post is fine. We have to track down the issue this way and from my side I can’t really point you a way to figure out what’s wrong, the plugin is extremely straightforward. If it’s bold, then the category for the permalink is indeed set-up and WP will use it if nothing comes in the middle of the way.
