Doesn't work in Plesk website

    I’ve installed Caldera Forms on several websites, using WordPress with a couple of themes. It seems to work fine except on one site which has a Plesk control panel where there are some errors. Error messages in the logs say the File Does Not Exist, source is Apache Error.

    Any help? Looks like a great utility but it doesn’t work.
