yes, the plugin should work on network installs as well. In fact, I just tested the current version after your report and it seems to be working for me.
What happens when you try to clean up the images inside the Media Settings? Also, are you aware that the plugin does not clean the default WordPress image sizes (thumbnail, medium, large)? Just mentioning it in case that’s what is confusing you.

The reason I don’t remove default sizes is that they would get generated anyway as soon as you visit your media screen (as they are all requested there) so it’s pointless to remove them just to have them all generated on first visit of the Media screen.
