Try updating to the latest version – we’ve not seen this issue. We’re utilizing the WordPress media library object to insert the images, are you having issues inserting images anywhere else?
Same thing here, broken image source. The image I am using is currently in my wordpress library. All I tried to do was send it to the widget. Using latest wordpress and a fresh download of your plugin….

Can you provide any more information than that? What is the path of the broken image? We’re running the plugin on dozens of client sites and do not have any problems.
No broken image – I just can’t load them into the widget, they just don’t appear in the list.
